По поводу проблем - слишком общий вопрос. Проблемы возникают всегда, когда идёт настройка оборудования от десятка производителей. Общий ответ - смотрите Conformance Statement'ы от DICOM устройств. Эти документы обязаны поставляться с любым DICOM устройством (аппаратом, принтером или программой). Иногда помогают в сложных ситуациях.

Опять же - слишком общий вопрос. Производителей множество. Кто как придумает - так PACS и будет работать. Более-менее однообразно PACS'ы работают на DICOM сетях. Опять же, DICOM описывает только среду передачи информации по сетям (еще и запись на разные носители). Как она будет хранится - зависит от производителя.
